Flexible Housing Pool (FHP) Case Manager

The Flexible Housing Pool (FHP) Case Manager provides comprehensive individualized services to all clients that are match in the Flexible Housing Program (FHP). This position provides comprehensive, individualized support services using a client centered harm reduction, trauma informed model for clients in the Flexible Housing Pool. Responsibilities include bi-monthly home visits to clients living in scattered site units across the Chicagoland area, accompanying clients to medical & benefits appointments and connecting clients (as needed) to community-based resources.

HUD Housing Case Manager (2 positions)

The HUD Housing Case Manager provides comprehensive individualized services to all clients that are match in the HUD/Specialized Housing Program. The Housing Case Manager will provide routine home visits and phone contacts, as established by the Director. The Housing Case Manager will assist individuals with identifying barriers to housing stability and engage in goal planning that aims to link individuals to the health and supportive resources that they need in order to achieve long-term healthcare and housing stability.
